There are a number of additional websites where caregivers can access support and resources for themselves and the person they are caring for. See below for a list of websites and a brief description of the website:

Caregiver Support During the COVID-19 Pandemic: Caregivers are more important than ever during the COVID-19 pandemic. The COVID-19 Community South West Region Pandemic Planning Table recognizes the increased strain and challenges of caregiving during this time. Here you will find a one-stop shop of information and resources to support caregivers in Grey Bruce, Huron Perth, London Middlesex, Oxford and Elgin.

Autism Ontario: Autism Ontario (formerly Autism Society Ontario) is the province’s leading source of information and referral on autism and one of the largest collective voices representing the autism community. We are made up of knowledgeable parents, professionals, and autistic self-advocates who can speak to the key issues that impact Ontario individuals with autism and their families and loved ones. Members are connected through a volunteer network of 25 Chapters throughout the Province of Ontario.

Big White Wall: This website is an anonymous peer support community that is online and accessible 24 hours a day.   Big White Wall supports mental wellness and has a variety of ways in which to connect on topics such as stress, anxiety, addictions, depression and diet. All content is moderated by clinical specialists to keep the community safe and productive.  No referral is needed.

Bounce Back: This is a free skill-building program for youth and adults to help manage symptoms of depression and anxiety through telephone coaching, workbooks and videos. A referral is needed for the program through your primary care provider or by self-referral.

Careblazers: Careblazers is meant to help the everyday hero caring for a loved one with dementia. If you feel alone, frustrated, and overwhelmed while caring for your loved one with dementia, Careblazers is here to give you helpful information and support right at your fingertips.

Caregiver Exchange: A website that provides practical tips and insights for family caregivers as well as access to an up-to-date, comprehensive listing of services across Ontario. It also features video interviews with experienced caregivers and a listing of local events.

The Canadian Caregiver Network: An online social community where you can connect with other caregivers and find a variety of resources including articles and videos.

Change Foundation: An independent healthcare think tank committed to improving the caregiver experience within Ontario’s healthcare system, including through the Changing CARE projects and work with young carers. A variety of reports, videos and resources are available online for caregivers and healthcare providers.

Huron Perth Health Links Coordinated Care Planning: Health Links is an approach to coordinated care planning that brings together your full care team together to develop a care plan based on you and your friend or family  members unique goals and needs.

MyChart: An online tool that allows a patient to access their health information from participating organizations at any time. Patients can give permission to caregiver(s) to access their record. Patients must register for MyChart at the hospital.

Ontario 211: Is a provincial gateway to community, social, non-clinical health and related government services. It can be accessed through their website or by calling 2-1-1.

Ontario Caregiver Organization: A provincial organization whose aim is raise awareness of the caregiving role, while providing information to caregivers about supports and resources. Use the Ontario Caregiver Helpline for 24/7 support at 1-833-416-2273.

Rehabilitation Services: Learn about rehabilitative care in the Ontario Health atHome - South West area and find local services and programs for you.

Respite Services:  This website provides information about respite services available across Huron and Perth counties and helps to connect families with respite support.

Southwest Primary Care Alliance: This website aims to serve as the internet home for health care providers, allowing practical sharing of electronic medical record forms, best practice tools, and patient friendly health literature.
